How we work with you
Referring a patient to us is simple. Just call us at 585-273-3760. For ALL referrals please include the following information if available:
- History and physical
- Echocardiography report
- Pulmonary Function Tests
- Recent office visit note
- Heart Catherization report
- Lab work (CMP, CBC, LFTs)
- For high-risk patients, our Critical Care Transport Vehicle is available for hospital-to-hospital transport.
- We return phone calls promptly and keep you informed on your patient's treatment and progress.
